Care Coordination

Providing enhanced holistic care coordination throughout Washington State, care coordinators work with and for our clients to provide enhanced care within the community.

Highly skilled care coordinators will integrate and coordinate all primary, acute, behavioral health, and long-term services and supports to treat the “whole-person” across the lifespan.

Care coordinators assist clients with:

  • Finding Doctors And Setting-Up Appointments
  • Coordinating The Care You Get From Different Providers
  • Helping You Understand Your Prescription Drugs
  • Helping With Food Services
  • Helping With Transportation To And From Medical Appointments
  • We Can Also Help With Housing And Social Services

Skilled nurses and community support members provide care coordination of medical, behavioral health, long-term services and support for individuals of all ages.

Care Coordinators support clients and families in improving their quality of life, assist with post-hospital care, managing multiple providers, getting patients to and from medical and self care appointments, and help clients get connected to available community resources.
